When you submit an application to Affinity, your personally
identifiable information will be shared only with the following:
- Me (Steve Freedkin), your Affinity
Representative.
- Affinity Corporation,
provider of the long-distance service.
- Your current telephone service providers, for the purpose of transferring
your service to Affinity.
- Affinity's partners/vendors as necessary to provide your service, including
billing and collection. Those parties will not market your information
or contact you, except as indicated in the next item. ...
- An independent service hired by Affinity to telephone you and verify your
information. The company cannot use your information for any other purpose.
- The organization you select to benefit with your calling.
- Affinity, like any business, must also disclose information if required
by law, for example in a law enforcement investigation.
Affinity does not disclose individually identifiable
customer information to other parties for marketing purposes.
As part of the customer qualification process, Affinity may
obtain your credit report from a credit bureau. Like other major phone companies,
Affinity may also disclose credit information to a credit bureau regarding customers
who fail to meet their payment obligations.
When you click SUBMIT, your information is sent over the Internet
to me. It is not encrypted. If you prefer, you may submit the information by
fax or telephone instead: see instructions at the top of the Service Request
Form.
